The selection expands out from the point of click using a flood fill algorithm. Any pixel within a certain tolerance of the start pixel is selected. If "antialias" is enabled, immediate neighbor pixels of the selected pixels are also selected, proportional to how close their color matches the start pixel.

You can click and hold, then drag sideways to change the tolerance and see the selected region change in real time.
